To form a South Dakota limited liability company you file Articles of Organization, Domestic Limited Liability Company with the South Dakota Secretary of State. The official South Dakota Secretary of State text states the fee:

What the form asks for
In the order of the official instructions for Articles of Organization:
- LLC name containing Limited Liability Company, Limited Company, L.L.C., LLC, L.C. or LC.
- Street address of the initial designated office (in or out of South Dakota), and a mailing address if different.
- A South Dakota registered agent: a noncommercial agent with an in-state street address, or a commercial agent with its CRA number.
- Name and address of each organizer.
- Whether the LLC is member-managed or manager-managed (list managers if manager-managed).
- Signature, printed name and date of each organizer.
